Pay by Credit Card
Once you have received your formal quote and have decided to bind coverage, you have the option of paying by credit cards.
If you would like to use this option, please download the credit authorization form below. Click on the button to download
a PDF Form. Type in your information in the blank spaces and your payment, and the form will calculate your Total.
Just a reminder that when you use your credit card, there is a credit card processing charge of 3.5%.
Remember that you must have Adobe Reader 8.0 or above in order to be able to type in the fields. If you do not have it

Payment Options
Here at Cossio Insurance Agency, we have created two simple options for our customers. It’s hard enough running a business, and that’s why we have made simple steps in order to help ease your worries.
1) We will accept full payment by the following options:
- A check mailed to us paid to Cossio Insurance Agency
- Visa, Mastercard, and Discover Credit Cards
2) You can finance the entire amount. All we ask is that you atleast pay 30 - 40%
of the amount for your downpayment. Then you can make installments up to 9 months.
